6. Copy of Hauling Contract with phone number to verify employment
7. Copy front & back of Drivers License
8. Proof of Business (DBA, LLC, or Schedule C
9. Copy of last years personal and business taxes
10. Proof of Residency (copy of utility bill or mortgage statement in applicants name)
11. Copy of last months cell bill (all pages)
12. Transmission serial number, engine serial number, ECM
13. Vehicle must pass a level 1 DOT Inspection and have proof.
