• Complete Invoice
• Year to date pay stub and (3) months Bank Statements
• Copy of Hauling Contract with phone number to verify employment
• Copy front & back of Drivers License
• Proof of Business (DBA, LLC, or Schedule C)
• Copy of last years personal and business taxes
• Poof of Residency (copy of utility bill or mortgage statement in your name)
• Copy of last months cell phone bill (all pages)
• Transmission serial number, engine serial number. ECM
• Vehicle must pass a level 1 DOT inspection and show proof
